FINGERPRINT CLEARANCE CARDS
Certified Employees must maintain a current and valid IVP Fingerprint Clearance Card (FPCC) issued by the Arizona Department of Public Safety. In addition, many other positions with FUSD require an IVP FPCC. Cards may take 4-6 weeks to process, please plan accordingly when renewing or obtaining your IVP FPCC.
Obtaining a Packet:
You will need an application packet for a FPCC. You can obtain a packet at FUSD HR or with the Flagstaff Police Department located at 911 Sawmill Rd., Flagstaff, AZ. In addition, you may call the Arizona Department of Public Safety at 602-223-2279 and request that a packet be mailed to you. There is excellent information on the AZ Department Education which includes how to get a packet for completing fingerprinting out of state. http://www.azed.gov/educator-certification/fingerprint-clearance-card-ivp/
Obtaining a FPCC Electronically: (NEW FASTER OPTION)
You may apply electronically at: http://fieldprintarizona.com/
If you are outside of Arizona, or do not have access to an internet connected device, you may alternatively obtain a packet directly from DPS by either calling (602) 223-2279 or faxing your request to (602) 223-2947. Office hours are Monday through Friday from 8:00 a.m. to 5:00 p.m. The electronic application will potentially reduce processing time by three to four weeks.
The fee for the FPCC is $67.00 and the processing fee is $6.00 when obtained at the FPD. Payment must be in the form of money order, cashier's check or business check.
Those who have a current FPCC may obtain a renewal without having to have their prints retaken. Please follow the directions on the application packet under the heading IVP Renewal Application. The fee is the same for renewals.
