Assistive Technology Referral Process
When is Assistive Technology considered?
1. When a child is referred for evaluation for special education services
2. At the annual IEP as determined by the IEP team
3. On the 504 plan
4. When the school AT team determines a child would benefit from assistive technology
What is the process?
The IEP team determines that the student cannot meet the IEP goals without the use of assistive technology so the following procedure is initiated:
Technology Referral Form (SE #8452) is completed
If the AT team does not have the expertise to recommend AT solutions, the district-wide AT Coordinator should be contacted to assist with the evaluation process (Jane Scott, ex. 5519)
The Assistive Technology Assessment process is followed:
1. The AT team convenes to discuss the student's need for AT. This need must be based on the student's IEP goals.
2. The AT team completes the Assistive Technology Consideration form to determine the needs of the student that cannot be met without the use of AT.
3. The AT team documents the AT meeting on the Meeting Notes form.
4. The AT team recommends appropriate AT solutions that can be implemented for the student.
5. AT recommended is obtained for trial use if appropriate.
6. The AT team documents the follow up plan for assessing the appropriateness of AT solutions on the Assistive Technology Follow Up form.
7. AT recommended for purchase is forwarded to Jane Scott, AT Coordinator, at SHS, ext. 5519, with a completed AT documentation and justification for purchase of equipment. Jane will determine if the equipment being requested is already available in the district for use by the student and if not, she will forward the request to Michelle Moore, Support Services/Admin. Center for purchasing
8. A follow up meeting is scheduled to evaluate the AT being used and to make further recommendations
Who implements the Assistive Technology?
The primary provider is responsible for monitoring and using the assistive technology
The AT Coordinator (Jane Scott, ex. 5519) is available to assist with implementation and training for the student or the staff as needed
How often do we review the student's Assistive Technology needs?
Periodic review can take place anytime as needed and documented on the Meeting Note form and the Follow Up form
At the annual IEP or 504 review
At the initial evaluation or re-evaluation
