These documents represent the User Agreements for using FUSD's Electronic Information Systems. EIS includes networks (including the Internet), databases, and any computer-accessible source of information, whether from hard drives, tapes, compact disks (CDs), floppy disks, or other electronic sources. All potential users of the EIS must sign the appropriate document before they can use the EIS. Click on the appropriate link below for a downloadable copy. EIS Employee Users' Agreement - For ALL employees of Flagstaff Unified School District, including Substitute Teachers. Return signed form to Human Resources.

Guest User's Agreement - For all other users of FUSD EIS. Return signed form to the EIS supervisor of the facility where you use the EIS.
