Health Services

Immunizations required for school entry according to Arizona law (A.R.S. statute 15-872) for the 2006 – 2007 school year.

PRESCHOOL:

1. DTaP/DTP (Diphtheria, Tetanus & Pertussis) – 4 doses
2. IVP/OPV (Polio) – 3 doses
3. MMR (Measles, Mumps & Rubella – 1 dose (must be given on or after 1st birthday)
4. Hib (Haemophilus influenzae type b) – 3 to 4 doses (One dose if first one given at 15+ months)
5. HBV (Hepatitis B) – 3 doses
6. Varicella (chickenpox) – 1 dose

KINDERGARTEN, FIRST, and SECOND graders :

1. DTaP/DTP (Diphtheria, Tetanus & Pertussis ) – 5 doses (4 doses if 4 th given at 4+ years)
2. IVP/OPV (polio) – 4 doses (3 doses if 3 rd given at 4+ years)
3. MMR (Measles, Mumps & Rubella) – 2 doses
4. Hib (Haemophilus influenzae type b) – Not required if over 5 years old
5. HBV (Hepatitis B) – 3 doses
6. Varicella (chickenpox) – 1 dose

THIRD, FOURTH, FIFTH and SIXTH graders :

1. DTaP/DTP (Diphtheria, Tetanus & Pertussis) – 5 doses (4 doses if 4 th given at 4+ years)
2. IVP/OPV (Polio) – 4 doses (3 doses if 3 rd given at 4+ years)
3. MMR (Measles, Mumps & Rubella) – 2 doses
4. Hib (Haemophilus influenzae type b) – Not required if over 5 years old
5. HBV (Hepatitis B) – 3 doses

SEVENTH and EIGHTH graders :

1. DTaP/DTP (Diphtheria, Tetanus & Pertussis) – 5 doses (4 doses if 4 th given at 4+ years)
2. Td or Tdap booster – 1 dose required 10 years after last DTaP/Td
3. IVP/OPV (polio) – 4 doses (3 doses if 3 rd given at 4+ years)
4. MMR (Measles, Mumps & Rubella) – 2 doses
5. Hib (Haemophilus influenzae type b) – Not required if over 5 years old
6. HBV (Hepatitis B) – 3 doses
7. Varicella (chickenpox) – 1 dose (2nd required if dose #1 given at 13+ years)

NINTH, TENTH, ELEVENTH & TWELFTH graders:

 1. DTap/DTP (Diphtheria, Tetanus & Pertussis) – 5 doses (4 doses if 4 th given at 4+ years)
2. Td or Tdap booster – 1 dose required 10 years after last DTap/Td
3. IVP/OPV (polio) – 4 doses (3 doses if 3 rd given at 4+ years)
4. MMR (Measles, Mumps & Rubella) – 2 doses
5. Hib (Haemophilus influenzae type b) – Not required if over 5 years old
6. HBV (Hepatitis B) – 3 doses

NOTE: The Varicella (chickenpox) immunization will eventually be required for every grade level. Two grade levels will be added every year until everyone is caught up.
