STUDENT INSURANCE PLANS

Myers-Stevens offers the following basic plans:

1.  Student Health Care Plan 
     (covers sicknesses and accidents)

 

2.  Full-Time (24-Hour) Accident Plans

3.  School Time Accident Plans

4.  Interscholastic Tackle Football Accident Plans

5.  Dental Accident Plan

6. Pharmacy SmartCard

For details about these plans please see a Myers- Stevens brochure.  All FUSD schools receive, from Myers-Stevens, shipments of these  brochures - with attached application forms - for enrolled students.

Brochures are usually distributed to students/parents during the first week of school.  If your child is enrolled later in the school year, please check with the school office for a brochure.  

Please read your brochure carefully.  If you have any questions, please call the plan administrator, Myers-Stevens, at (800) 827-4695, or at (949) 348-0656.  Bilingual representatives are available for parents who need assistance in Spanish.
