2020/2021 Immunization Requirements:
ALL incoming 6th graders already 11 years old will need proof of the Tdap and MCV vaccinations to start school.
For ALL newly registering students, I will need a copy of the entire shot record. If they are already 11 years old or older their immunization record will also need to include proof of the *Tdap and *MCV
*(some states don't mandate them, AZ does).
If your child was born in AZ or has been here for an extended period of time, check in this database for your child's records and, or register if you have not already. Your primary care provider (PCP) is the one who should be putting your child's record here. If they are not here, call your PCP to ask them to put them here once they are updated.

Immunizations: All school required and CDC recommended vaccinations are available. Please bring immunization records and insurance card, if applicable. Childhood immunizations are available via the Vaccines for Children (VFC) program or through insurance. Self-pay options are offered on a sliding scale and no one is denied services due to inability to pay.
Appointments available Tuesdays and Wednesdays from 8 am to 5 pm.
Sports Physicals: Cost is $30 payable by cash, check or charge. Sliding fee scale available. Minors must be accompanied by a guardian. The Arizona Interscholastic Association form is available at https://www.coconino.az.gov/1976/ Health-and-Wellness-Clinic. If possible, please complete pages 1 and 2 and bring a copy to your appointment.

If your child has a CHRONIC HEALTH condition, please stop by the Health Office during registration or call Nurse Avery to make an appointment (527-5502).
Students with school-related injuries or emergencies should report to the nurse's office immediately. If the nurse is unavailable, an administrator should be notified. There will be a note on the Health Office door directing the student to the office when the nurse is out of the building/office.
*All students must have emergency telephone numbers on file with the nurse and report any emergency telephone number changes immediately to the data clerk's office (counseling office secretary)* (527-5520)
!For the protection of all, students may not bring over-the-counter or prescribed medications into the nurse. *See below for self-carry protocols*
When medications are necessary during school hours, the school nurse will assess the medicine and may administer a medicine according to the following requirements:
- Tylenol (non-aspirin) 325 mg tabs and Motrin (Ibuprofen) 200 mg tabs * The Health Office has these two medications on-site and available for students* These two medications may be administered to your child during the day ONLY if there is a parent-signed district consent form on file, either electronically, or a hard copy is in the nurse's office. The nurse is not allowed to receive a verbal consent over the phone, it must be in one of the above forms.
The nurse will not be able to give more than the manufacturer's recommended dose range unless it is prescribed by your MD *which then must be brought in by the parent according to the below protocols*
- ALL Over the Counter medications MUST be brought into school by a parent, signed in by the parent on an FUSD medication form, and be in their original and up-to-date bottle which will be kept in the nurse's office so the student can access them as they need and as directed by the parent.
- Prescription medications may be administered once cleared by the nurse and only if the medication is in its original and up-to-date pharmacy-labeled container. A parent-signed district consent form must be on file in the nurse's office in order for the nurse to administer any prescribed medication. Medications must be brought in by a parent and kept in the nurse's office. For your child's safety, NO NARCOTICS should be given at school. If your child needs to be on a narcotic after a procedure they should be at home while they are still taking it.
*SELF-CARRY* Inhalers/Epi Pens/Diabetes Supplies: These prescribed medications/supplies may be carried by the student as long as there is the necessary FUSD form signed off by the MD and in the Health Office. Please, see the nurse for questions.
Daily at Home Screening for Students
Parents: Please complete this short check each morning with your student and report your child’s development of these symptoms to your school nurse (insert school nurse name and contact information). Your student’s information will be protected in accordance with FERPA and HIPAA.
SECTION 1: Symptoms
If your child has any of the following symptoms, that indicates a possible illness that may decrease the student’s ability to learn and also put them at risk for spreading illness to others. Please check your child for these symptoms:
Temperature 100.4 degrees Fahrenheit or higher when taken by mouth;
New uncontrolled cough that causes difficulty breathing (for students with chronic allergic/asthmatic cough, a change in their cough from baseline);
Diarrhea, vomiting, or abdominal pain; or
New onset of severe headache, especially with a fever.
SECTION 2: Close Contact/Potential Exposure
Had close contact (within 6 feet of an infected person for at least 15 minutes) with a person with confirmed COVID-19. Close contact may also include being sneezing on, coughed on, hugged, or kissed by someone with COVID19. Sharing drinks or utensils would also be considered close contact): OR
Had close contact (within 6 feet of an infected person for at least 15 minutes) with person under quarantine for possible exposure to SARS-CoV-2; OR
If the student/parent/caregiver answers YES to any question in Section 1 but NO to any questions in Section 2, the student would be excused from school in accordance with existing school illness management policy (e.g., until symptom-free for 24 hours without fever reducing medications).
If the student or parent or caregiver answers YES to any question in Section 1 and YES to any question in Section 2, the student should be referred for evaluation by their healthcare provider and possible testing.
Students diagnosed with COVID-19 or who answer YES to any question in Section 1 and YES to any question in Section 2 without negative test results should stay home, isolate themselves from others, monitor their health, and follow directions from their primary care provider or local health department. Students should be permitted to return to school in line with current CDC “When Can I Be Around Others” (e.g., 24 hours without a fever, 10 days after symptoms onset). A negative test or doctor’s will not be required for return. Questions regarding return to school should be jointly decided in consultation with parents or caregivers, school personnel, and the student’s healthcare provider. Students and their families should be advised that the local health department may contact the family for contact tracing. If contacted, families should notify the contract tracer that the student attended school.
Students that answer No to Section 1 but Yes to any component of Section 2 (for example: if a student is asymptomatic but has had close contact to a confirmed case of COVID19) the current recommendation is for close contacts to quarantine at home for 14 days following last known exposure date to monitor for symptoms, even if they receive a negative test result while in quarantine.)
Students who are excluded from school should be afforded the opportunity, as soon as feasible when they are well enough to participate in classwork, to make up any missed classwork without penalty in order to reduce mental or physical anxieties about missed academic opportunities.
FUSD does not provide medical insurance coverage for school accidents. Parents/guardians are responsible for all medical bills if their child is hurt during school activities and athletics. Student accident/health insurance plans are offered to help pay for any medical bills and information is available at the school office.
A variety of coverage options are available. The Student Health Care and High Option 24-hour Accident plans are recommended for those students with no other insurance because these plans provide the greatest coverage when injuries occur. If you do not have other health coverage, student insurance also may be used to pay those eligible charges not covered by other insurance.
