Student Records Requests

  • Flagstaff Unified School District is the Custodian of Records for all students who have attended schools in the district.

    To request Student Records (i.e. high school transcripts, medical) please print, complete, sign, and return the Authorization for Release of Pupil Records (via mail, fax, or email) to our District Records Clerk, Madelyn Trillo.
