For students and families interested in registering for NADL full time, each step must be completed to confirm enrollment.

Required documentation includes the following materials:
- Valid Arizona driver's license
- Proof of Residency (Real estate deed or mortgage documents, Property tax bill, Residential lease or rental agreement, Water, electric, gas, cable or phone bill, bank or credit card statement, Payroll stub)
- FUSD Tech Agreement
- Child's Birth Certificate
- Home Language Survey
- Pupil Registration Form
- Residence Form
- Academic Contract
- 506 Form (certificate of tribal enrollment issued by a recognized American Indian Tribe (If Applicable))
**If the student is taking a make-up class, credit recovery, or is trying to get ahead on credits towards graduation, then complete only Step 1 and complete the Registration Fee form. You do not need to submit other registration information.**
