For enrollment verification, print or take a screenshot of your child's schedule from your FUSD ParentVUE account or StudentVUE account. *ParentVue accounts were provided at registration as well as in emails from school.

Contact Leanne Bennett - Registrar at 527-5530 if you need to re-activate your SMS ParentVUE account.

Applications will be made available after August 27, 2021.

*Click on "Application Portal" on top left hand corner of your screen to apply instantly. 

*Be prepared to upload required documents.

     1.  Child's Certificate of Indian Blood

     2.  Child's valid social security card.

     3.  Documentation of school attendance on letterhead.  We were informed that a school class schedule will work. *The class schedule should have the student name, the current school year, the name of the school, and possibly an address and phone number for the student. Print or take a screenshot of your child's school schedule from your FUSD ParentVUE or StudentVUE account.  *ParentVue accounts were provided at registration as well as in emails from school.  Contact Leanne Bennett - Registrar at 527-5530 if you need to re-activate your SMS ParentVUE account.

     4.  Complete W-9 that is signed by the application.

*Click on download application to print.
