Greetings Everyone!
For INCOMING 6th graders:
ALL students who are 11 years old will need proof of the Tdap and MCV immunizations (these are the 11 year-old shots mandated by the state of AZ for school attendance).
I have been sending out emails, making phone calls to many already, as many students are past due. I must take care of this over the summer due to the time this task requires. PLEASE, make every effort to get these needed shots BEFORE school starts and scan to my email, or drop off to the front desk staff to put in my mailbox, so I can get your child off of the non- compliance list ASAP.
As per FUSD policy: "Students will not be allowed to begin school without up-to-date immunizations with the exception of students with an exemption on file or students covered by the McKinney-Vento Law. An immunization record signed by the health care provider must be on file with the school before a student may enroll."
If you have any concerns regarding this, please contact me so we can get any issues resolved, again, BEFORE school starts.
FOR ALL STUDENTS that are new to SMS:
I will need up-to-date immunization records which include: Tdap and MCV (11-year-old shots in many states). If your child has not gotten these shots, they will need proof of them to start school, so I will need proof of them BEFORE school starts. Feel free to contact me with questions.
Chronic Conditions: Any parent with a student that has a chronic medical condition/ medication that will need the nurses' attention during the school day, please contact me BEFORE school starts so we can clarify any needs that must be met. To respect others' time, registration lines are too long to discuss these concerns.
