NORMAL HOURS OF OPERATION

  • 6:45 am until the start of school
  • Dismissal time until 6:00 pm

TWO-HOUR DELAY

  • Morning FACTS will be held at each site from 7:30 am until school begins.
  • Afternoon FACTS will operate as normal on these days.

 SNOW CANCELATION DAYS:

  • FACTS will be closed when FUSD cancels school due to snow.

Parent/Guardian Guide for Non-School Day FACTS:

  • Non-school day FACTS must be enrolled separately.  
  • Parents are responsible for transportation to and from FACTS. The FUSD buses do not run on non-school days. 
  • FACTS does not provide lunch, so be sure to send your child with their own food.
  • Check-in times may be longer than normal during non-school-day FACTS programs. Be sure to plan extra time into your morning commute.  
  • Remember parents, you need to physically sign your children in & out at the front desk, and be prepared to show ID to FACTS staff.

FACTS will be CLOSED on other holidays not listed above.

Setting up your account does not enroll you in FACTS. It will save time when you do register by auto-filling many registration fields. You may find that during the course of the year you may need to make changes to your children's schedule. Please follow the FACTS Schedule Change Requests help sheet.  

Medication consent form
If your child requires medication of any type, this form must be completed and returned to the FACTS program before we can administer that medication. You can get the form here.

Medication requirements

  • Any medication must be in the original packaging.
  • The prescription must be for the student indicated on the consent form.
  • Expired medication will be returned and can not be used.
  • Over-the-counter medication can be provided for FACTS as well.
  • Medication dosage instructions provided must be in-line with manufacturing use instructions. 

At FACTS, we are proud to offer a nutritious and tasty snack every day before 4pm. We follow all National School Lunch Program (NSLP) guidelines. If your student has special dietary requirements, we encourage parents to send a snack with their child. If this is not available, please communicate restrictions with your site's leadership staff. Students are encouraged, but never required to eat the provided snacks. Snacks are never withheld from any student as punishment. FACTS does offer a variety of cooking clubs within most sites. However, the food during clubs does not substitute the daily snack.

Consequence Philosophy and Policy

The FACTS program ascribes to the philosophy that every choice has a consequence. This is something that we hope to pass on to the students who join us at FACTS. When students make positive choices, they should earn positive consequences, such as praise or rewards. The opposite is true when FACTS students make poor choices. We will make every effort to make certain that the consequences of choices are tied to the choice itself.  An example of consequences directly related to student choices is, if a student chooses to litter, he or she may be asked to do litter removal from program areas. We will communicate with parents if their child has negative consequences. Extreme or continuous behavior issues may result in the students’ expulsion from the program. Expulsion is determined by the site’s FACTS Specialist/Assistant Specialist along with the Program Coordinator.

 

Level 1 Negative Choice

Choices that will result in an automatic FACTS Incident Form include, but are not limited to:

1)    Consistently not following directions with the intention of being disruptive.

2)    Violating technology guidelines.

3)    Inappropriate language.

4)    Unsafe Behavior (unsafe to themselves, other students or staff).

 Three FACTS Incident forms may result in a suspension for an amount of time determined by the site's FACTS Specialist/Assistant Specialist and/or FACTS Coordinator.

 

Level 2 Negative Choice

Choices that will result in an automatic suspension include, but are not limited to:

1)    Physical violence or the threat of physical violence.

2)    Use of sexually explicit, racially derogatory, or gendered defamatory language or harassment.

3)    Stealing and vandalism.

4)    Leaving the FACTS area without permission (running away from the group or hiding from FACTS staff).  

If any student receives a suspension three times throughout the program, this may result in an expulsion from the program for an amount of time determined by the site's FACTS Specialist/Assistant Specialist and FACTS Coordinator.

 Parent Guardian Agreement

  • Children must be between 5 and 12 years old and be able to use the restroom unassisted.
  • An authorized adult must walk my children up to the FACTS entrance and sign them in in the morning. An authorized adult must also walk up to the FACTS entrance to sign them out.
    • Anyone picking my child up from FACTS, including myself, is required to show their photo ID to any FACTS team member upon request.
    • Only adults authorized to pick up my child will be allowed to do so. The primary account holder can edit the authorized pick-up list by logging in to their Eleyo account.  
  • No external adult will be allowed to enter the school building unless invited by a FACTS team member. This includes parents and all authorized emergency contacts.    
  • FACTS staff have permission to sign my child out of FACTS in the morning before the start of school, and to sign my child into FACTS at the end of the school day.
  • My child’s behavior can result in suspension or expulsion. Use of this measure is at the discretion of the site Enrichment Specialist and FACTS Coordinator. Where appropriate, other age-appropriate behavior management techniques will be used prior to suspension or expulsion. FACTS Behavior Policy is available by going to the FACTS website
  • If my child has not been picked up within 30 minutes after the facility closes, law enforcement officials will be called to pick up my child.
  • I agree not to hold any person(s) or organization connected with FACTS and Flagstaff Unified School District, responsible for injuries or illness sustained by my child on the normal course of play. I understand that the said organization does not provide medical insurance coverage for my individual child. I am responsible for the medical bills if my child is injured or becomes ill.
  • FACTS provides a nutritious snack before 4:00 pm daily. Snack menus are posted on the FACTS website.

    • If my student has special dietary needs, I am required to send a snack with my child when the scheduled snack does not meet their needs.

    • All field trips require a separate permission form to be completed by the parent or guardian.
    • My child must be dropped off and pickedup at the program site, not at any field trip location.
    • I must have my child signed into FACTS by the time listed on the field trip permission form in order for my child to attend.
  • FACTS may not be able to accommodate all dates requested, and it is the parent/guardians' responsibility to review their child's schedule after they receive an approval notification.  
  • Any schedule change request must be made at least one week prior to the date you wish to add or remove. FACTS Schedule Charge instructions and information can be found on the FACTS Registration Information page.
  • Drop-in days are requested schedule changes that occur within 1 week of the requested date.  
    • Drop-in for morning FACTS must be received by 5:30 the night before and will be approved or denied by 6:00 pm.   
    • Drop-ins for afternoon FACTS on standard school days must be submitted by 11 am (10 am at PdH) and will be approved or denied by 12 pm (11 am at PDH).
    • Drop in for afternoon FACTS on half days must be made by 9 am (8:30 at PdH) and will be approved or denied by 9:30 am (9 am at PdH).

Camp Iwannago Registration live now!

Registration for our annual summer camp, Camp Iwannago, is live now!

We are going to offer swim lessons once again this summer. Lessons will start June 10th, and go for three week sessions on Tuesday, Wednesday, and Thursday mornings. These are on a first come, first served basis, and students must be registered in Camp Iwannago to attend. Campers will need to be enrolled each day of the lessons, otherwise students who are enrolled each day will have priority of attendance.

Finally, as with every summer, we will be embarking on some amazing field trips to explore what makes Flagstaff and Northern Arizona such a beautiful, amazing, and fun place to call home.

You will be notified as your registration is approved or if you've been put on a wait-list, keep in touch with the FACTS Supervisor at your contracted sites for this information.
