Benefits Overview - Open Enrollment is May 20, 2024 through May 31, 2024

Welcome to your FUSD benefits!
As an employee working 30 hours or more per week, or .75 FTE, you are eligible for benefits.
You are required to enroll in medical, dental, vision and life coverage. However, you do have the option of waiving medical coverage if you can provide proof of other coverage and complete the Waiver of Medical Coverage form on the enrollment website. If you choose to enroll dependents in any of your benefit plans, you will be responsible for paying 100% of their coverage. Click here to see the 2023-24 Employee Benefits Rates.
You can also enroll in voluntary plans, including additional life insurance coverage for yourself; life insurance for your spouse or child(ren); and Short-Term Disability. You will be responsible for paying 100% of the cost of any voluntary benefits. If you elect to enroll in Short-Term Disability you must complete the enrollment forms found on our website https://www.fusd1.org/Page/14293 and need to be sent to Assurant to the address on the form.
You can save each month on your medical benefit costs and copays by participating in the NAPEBT Wellness Incentive Program and utilizing the Vera Whole Health Clinic - Flagstaff. Please remember – the Vera Whole Health Clinic is NOT a walk-in clinic; you MUST make an appointment!
